Health Services

NSHA High School utilizes the Magnus Health Portal for your child’s annual health information. We ask that you enter the health information required within your Magnus Health account for each child attending NSHAHS. Further instructions are attached.

Timely submission of health information and other forms is vital so that preparations can be made prior to the first day of school. We estimate that entering this information for the first time will take you twenty to thirty  minutes per child, depending on circumstances. You will begin receiving weekly reminder emails from Magnus Health notifying you that there are outstanding items in the online Student Medical Record (SMR). These emails will list any incomplete requirement(s) or any existing requirement(s) that need to be updated.

If you are a first time user, simply login to Magnus and locate the items marked with a “To Do” flag to take action.

Return users will simply provide updates to the record as needed. All your data is stored securely.  Please click on the buttons below to access:

Please note the following forms are REQUIRED for admission to NSHA:

Additional Required Medical Forms:

  • Health History (parent completes)
  • Physical Exam Form (signed and stamped by physician)
  • Dental Form 
  • Confidential Waiver (parent completes)
  • Record of Immunizations (signed and stamped by physician)
  • NSHAHS Immunization Compliance Agreement (parent completes)
  • Medical Release (parent completes)
  • Emergency Contact Form

If applicable, complete the appropriate Action Plans (Asthma, Allergy, Diabetic Medical Management Plan, Seizure, etc.) as well as the Independent Provider and Parental Consent form for Independent Self-Carry and Self-Medication. This form applies only to epi-pens, inhalers and diabetic management. These Action Plans must be dated after the last day of the current year to be valid for the upcoming year. 

Please plan to complete Magnus forms for each enrolled child by July 15, 2023.
